- Your contact information, including your full name, mailing address, telephone number, and email address.
- A statement that you have a good-faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law.

Our Response
Upon receiving a valid notice, we will remove or disable access to the allegedly infringing material within a reasonable time and, where appropriate, notify the party responsible for posting it. We reserve the right to request additional information before acting, and we may reject notices that are incomplete or clearly invalid.
Counter-Notice
If you believe your content was removed or disabled by mistake or misidentification, you may submit a written counter-notice to our designated agent that includes all of the following:
- Your physical or electronic signature.
- Identification of the material that was removed or disabled and the location at which it appeared before removal.
- A statement, under penalty of perjury, that you have a good-faith belief the material was removed or disabled as a result of mistake or misidentification.
- Your full name, mailing address, and telephone number, and a statement that you consent to the jurisdiction of the appropriate court and will accept service of process from the party who filed the original notice.
If we receive a valid counter-notice, we may restore the removed material in accordance with the DMCA, unless the original complaining party notifies us that they have filed a legal action seeking to restrain the allegedly infringing activity.
Repeat Infringers
We may, in appropriate circumstances and at our discretion, restrict or remove content and take action against parties who are found to be repeat infringers.
